What I help with
Portals are free and useful, and they are also one more login, one more app and one more security code. Most of the calls I get are about the same handful of problems.
Getting signed up
Finding your activation code, verifying your identity and choosing a username and password you will remember.
Locked out
Forgotten passwords, codes that never arrive, and accounts that stopped accepting your login.
Using it day to day
Reading test results, booking visits, requesting refills, and sending a message to your doctor's office.
Family access
Helping a son or daughter follow a parent's care the proper way, with the right forms and the patient's permission.
Phone and tablet
Installing the app on your iPhone or iPad, turning on Face ID, and keeping notifications under control.
Too many portals
One list of every portal you have, what each is for, and how to sign in. Kept organized and private.
Hackensack Meridian Health MyChart
Hackensack Meridian Health runs its patient portal on MyChart. It is free for current patients of the network, and it is where you will find results, visit summaries, medications and billing from your HMH doctors and hospitals.
- Account setup. We start from the sign-up page, answer the identity questions together, and set up your two-step security so you are not locked out later.
- Activation code or no code. If you never received a code, or it expired, we request access the way the portal intends.
- Forgot your password. We reset it, then write down how you will sign in next time in a place that makes sense for you.
- Messaging your doctor. Secure messages are for non-urgent questions. I show you how to write one, attach what you need and find the reply.
- Appointments, refills and results. The three things most people open the portal to do, walked through slowly until they feel routine.
- The phone app. Installed, signed in and ready, with notifications set so you hear about new results but not every small update.
Helping a parent or family member
When someone wants to follow a loved one's care, HMH uses proxy access. It is granted after an authorization form is completed and processed.
Adult to adult
For a patient age 18 or older. Both the patient and the proxy complete and sign the HMH MyChart Adult Proxy Form. The patient can end access at any time, in writing or from inside their own account.
For a minor
Patients under 18 need a signed proxy form. Access ends automatically on the patient's 18th birthday.
The patient stays in charge. I help families understand the forms and get them in order. I do not sign in as someone else or work around the portal's own safeguards.

Other portals, too
Each health system, practice and lab keeps its own separate account, so a login for one will not open another. If you see an "invalid login" message at the right website, it is often simply the wrong portal. I help sort out which one you need.
- Hospital and health system portals across Monmouth County and beyond
- Specialist and primary care practice portals cardiology, eye care, dermatology and more
- Lab and imaging portals where results and reports are posted
- Pharmacy accounts refills, pickup alerts and prescription history
- Insurance and Medicare accounts claims, statements and coverage cards
